Working as a plastic surgeon in Saudi Arabia in 2026: salaries, SCFHS homologation and hospitals hiring

Four key drivers:

1. Middle class with high purchasing power: Saudis have per-capita income among the highest in the world, no income tax, with high and growing demand for aesthetic surgery. Rhinoplasty, breast surgery (augmentation and reduction), liposuction, abdominoplasty and BBL are among the most requested procedures.

2. Social opening since 2017: social reforms have normalized aesthetic surgery for Saudi women and, increasingly, men. The social stigma has practically disappeared.

3. Regional medical tourism: patients from across the Gulf (Kuwait, Bahrain, Qatar, UAE, Oman) travel to Saudi Arabia for procedures with certified international surgeons.

4. Vision 2030 and reconstructive surgery: expansion of pediatric reconstructive surgery services (cleft lip and palate, congenital malformations), oncologic reconstructive surgery (post-mastectomy breast, head and neck) and microsurgery in large hospitals.

Plastic surgeon, 3–5 years: SAR 45,000–60,000/month (USD 12,000–16,000) + procedure bonuses.

Every plastic surgeon who wants to practice in Saudi Arabia must complete official recognition by the Saudi Commission for Health Specialties (SCFHS). It is the legal license to operate. Without it no Saudi hospital or private clinic can hire you, no work visa is issued and no medical malpractice insurance covers you.

The process has 4 pillars:

3. SCFHS Classification: you are assigned a rank (Specialist, Senior Specialist, Consultant) that decides which hospitals can hire you and at what salary.

4. MUMARIS+: activation on the Ministry of Health digital platform. Without an active MUMARIS+ you cannot legally operate.

Official specialty in Plastic, Aesthetic and Reconstructive Surgery (MIR in Spain, formal certified residency in Latin America, European or American plastic surgery board). At least 2 post-residency years for Specialist.

At least 5–7 years for Senior Specialist. At least 10 years + head-of-service role for Consultant.

Clinical English B2/C1. Arabic not required.

Highly valued (critical for salary): subspecialty fellowship (microsurgery, cranio-facial, hand, reconstructive breast), documented photographic surgical portfolio (before/after with consent), annual surgical log (number of rhinoplasties, breast, liposuctions, abdominoplasties, facial, reconstructive), scientific publications, ISAPS/ASPS/EASAPS membership.

There are two main contract types for plastic surgeons in Saudi Arabia:

1. Contract with a large hospital (Sulaiman Al Habib, Saudi German, King Faisal, Cleveland Clinic): high base salary (SAR 45,000–95,000/month) + productivity bonus per procedure. Mixed aesthetic + reconstructive cases. Full team (anesthesia, surgical nursing, post-op follow-up). Medical malpractice insurance paid by the hospital. Stability and prestige.

2. Contract with a premium private aesthetic clinic (Al Olaya, Al Nakheel, Al Rawdah): more moderate base salary (SAR 30,000–50,000/month) + very high productivity bonus per aesthetic procedure. Possible partner model (equity participation). Almost exclusively aesthetic cases. High margins. Higher surgical volume. Ideal for plastic surgeons with a premium aesthetic-oriented profile.
